We are a care agency providing support to individuals with dementia, physical and learning disabilities and others requiring care and support at home and in the community.

With over 17 years’ experience in the health and social care sectorwithin the UK,our mission is to be recognised as an excellent, friendly, flexible, person-centred care services provider, promoting independence, choice, privacy, dignity, transparency, and respect for all our service users and their families.

We operate and use multi-disciplinary team approaches. The Secareserve team is made up of social workers, mental health nurses, occupational therapists, physiotherapists, learning disability nurses and general nurses coming together to provide a unique and strength-based approach service for individuals who need support to live in their homes as well as giving a reablement service where care receivers or patients’ independence is promoted.

All our staff, including the support staff, are adequately scrutinised as part of our rigorous high standard checks to ensure that our service is of excellent quality and our staff practice safely.Additionally, as a multi-disciplinary team, we aim to deliver person-centred, coordinated services based on the Care Act 2014 and all statutory legislations.

SECARESERVE is a strong advocate of person-centred and strength-based approaches to providing care. Our assessment team use strength-based approach tools that are geared towards understanding best the needs of each individual, their strengths, aspirations, interests, wishes and views. Our assessments are also based on the core principles of the Care Act 2014 and key legislations.

From the point of referral or contact with us, our multi-disciplinary team will screen the referral or communication to ensure we can provide the service and have the appropriate staff for the individual. A risk assessment is also performed. Following a review of the outcome of the assessments, a support plan will be completed with a team allocated to implement the plan according to the needs of the individual.

This process can also be adapted to meet crises or emergencies and will be reviewed daily with a further 6-week and annual reviews completed.

As part of our service delivery, the following measures will also be observed:

  • We will put in place extensive guidelines and risk assessments to ensure individuals using the service are always protected from abuse and that we have reporting procedures in place if they arise. These will include guidelines in the administration of medicines, use and handling of finances, behaviour management, whistleblowing, and reporting. These will ensure that all individuals are supported positively with empathy using suitable structures (including methods of communication) and creating a positive working environment.
  • We will be working collaboratively with other professionals, e.g., GPs, dieticians, community teams, behaviour teams etc., to ensure that a consistent service is always provided to each person we support.
